Academy Webinar: Malnutrition Care Beyond the Hospital Walls

Medicare's Value Based Payment program provides incentives for high quality and cost-effective health care. Medicare payments to Accountable Care Organizations, Medicare Advantage plans, and PACE (Programs of All-Inclusive Care for the Elderly) programs are influenced by Hierarchical Condition Categories that are part of the VBP program. Accurate coding of chronic diseases, like obesity and malnutrition, helps forecast expected cost of care and improve a provider's quality score. Additionally, adequate malnutrition documentation and coding improves a patient's transition throughout the care continuum to ensure appropriate nutrition services are provided.
